How do I send a referral and find a provider?

For: Caseworker

Send a referral from a participant's pathway and track it through to whether the person actually connected. Referral Partners is the working list your team refers to, and a directory result you import lands there before anyone has vetted it. Search directories and Marketplace draw on public government sources for providers you have not added yet.

Referring a client

On a service line’s Referrals & Milestones tab, add a referral to a Marketplace listing or a directory result. Track delivery states, log check-ins, and record the outcome. On the Pathway screen, a referral still marked Referred or Connected blocks Complete case until you close it.

The provider response link

Each referral can generate a tokenized link to send to the receiving provider. The provider opens it (no account needed), sees the referral context, and responds: accepted, updates, or a close-out with a note. Responses land back on the referral.

Search directories

On Referral Partners (staff) and the public Find Providers page, search six live national sources:

Source

What it covers

findtreatment.gov (SAMHSA)

Mental health and substance-use treatment facilities

CareerOneStop: American Job Centers

Workforce centers

CareerOneStop: Training providers

Vocational and training programs

CareerOneStop: Re-entry programs

Re-entry services

NPPES NPI Registry (CMS, the Centers for Medicare & Medicaid Services National Plan and Provider Enumeration System)

Behavioral health and medical organizations

data.hud.gov (HUD, the U.S. Department of Housing and Urban Development)

HUD-approved housing counselors, serving Housing and Financial Stability

Search by zip code and radius. The radius applies to the sources that support it: the NPPES registry has no radius search and matches on the ZIP code area instead. On both pages, workforce results display the required CareerOneStop/DOLETA/DEED attribution. Find Providers is free and public: no account needed.

Marketplace and Referral Partners

Marketplace is a browsable directory of organizations by category tile, open to administrators only. Its listings carry a notice that Lift Right does not necessarily endorse them or have a working relationship with them. Caseworkers reach the same listings through Browse providers to refer on a service line. Referral Partners is the working list your org refers to, including importing directory search results as partners. Import & refer on a service line adds a directory result to that list in one click, before anyone has vetted it, and the referral row warns that the contact has not been confirmed.
